How much crimp are you using?

I had a Baer Premier II that did the exact same thing. I could see a "smile" in the brass where it would wedge into the barrel. Not an exact comparison because mine was rebarreled with a Kart and I was using 200gr LSWCs. Increasing the crimp to .466 fixed the problem.
I'm not sure what magazines your using, but I have similar problems with the Metalform magazines that have the rounded followers. Chip McCormick Shooting Star magazines run 100% for me so that's what I'm using.
Forgot to mention -- I am also seating to .925.

More crimp and seating at or even lower than .925 off the shoulder solved it completely. I’m at .920 and guessing .463 or so on the crimp.
(I was originally at .935 and minimal taper crimp.)

May also consider magazines whose lips are tuned to release the round a bit sooner so it can feed into the chamber straighter.

....in addition to above comments, check extractor tension. If it is too tight it retards the base of the brass from sliding up the breach face behind the extractor claw/hook preventing the cartridge from aligning with the barrel in time as the slide goes forward.
Remove slide from gun, remove barrel from slide. While holding slide horizontal, take a loaded round and slide up from bottom of slide under the extractor onto the breachface. With round in slide, behind extractor, rotate the slide like an axle. The round should remain in place behind the extractor during this process. You should see a tad bit of movement in the round as the slide is rotated. If it is held fast with no movement, then the extractor is likely too tight, thus not permitting the round from sliding upward in the rear (rim) to align with the barrel as the slide goes forward.

Changing the timing of the magazine will likely get it done as well as checking extractor tension. I would also polish the lower radius behind the extractor hook.
